"Welcome Home" Never Felt Better
If you've ever been in the hospital or a rehab facility, did you know that the first 10-12 weeks back home are the most critical of all and represent the period when most re-admissions occur? Transitioning back home safely can also be quite a challenge because most patients are under the false impression that "Home Health" (paid for by Medicare) will provide them with what they need when they get home. Home Health only provides skilled medical services that typically include just 2-3 visits per week for a limited length of time. Who will prepare your meals, do your laundry, help you get dressed, attend to your bathing and hygiene, help keep you from falling, run errands, and do light housekeeping? Well, that's where we come in!
We'll not only get you back home safely and provide you with all these important personal and homemaking services, and more, but our Safety Net Program will also help prevent your re-admission. Let us show you how good Welcome Home can feel!
